INJURY INFORMATION
Injury reporting & insurance claims
The safety and wellbeing of all participants is a priority at Surfcoast Basketball Association. If you sustain an injury during an SBA competition or program, please ensure it is reported as soon as possible.
Reporting an Injury
If you are injured during an SBA competition or program, you must:
- Notify the Competition Supervisor (CSO) on duty as soon as possible.
- Assist the CSO in completing an Injury Report Form by providing accurate details of the incident.
Completing an Injury Report at the time of the incident is important, even if the injury appears to be minor.
Making an Insurance Claim
If you wish to make an insurance claim for your injury, you will need to:
- Download the Basketball Australia Injury Claim Form from the V-Insurance website.
- Complete the participant section of the claim form.
- Forward the completed form to Surfcoast Basketball Association so we can complete the Association section before it is submitted to the insurer.
Important Information
Please note that Surfcoast Basketball Association can only support an insurance claim where an Injury Report Form was completed at the time of the incident. If an injury was not reported to the CSO and recorded on the day it occurred, SBA may be unable to verify the incident in support of your claim.
